does anyone know where I can get a basic budgeting form that I can put in all the information in the headers/titles myself. Please let me know on here. thank you in advance.

I suppose you haven't tried out the Simple budgeting tool prepared by this community. Just try it; i did so and it's very easy to figure out my monthly costs and earnings.
