It includes discount and origination points, settlement fees, escrow deposits for taxes, title insurance, mortgage insurance, brokers' fees, commissions and hazard insurance. Both the seller and buyer sign on it at the time of closing. This standard form is also called HUD-1 Statement published by the Department of Housing and Urban Development (HUD).
