Adding a name to a home/land deed

It is in his mothers name (property next door is his, just in her name). What do we have to do to get his name on there too. His mom is ill & we fear the worst. We were told we need an attorney. I agree with Mike above that you can use a quitclaim deed bit it need to be signed by his mother as she the owner. And after signing the quitclaim deed she will no be the owner of the property.
