Is there any simple way that I can find the forms for Illinois and file a quick claim deed to add my wife's name to property title - without needing a lawyer?

You can view a sample quitclaim deed form for the state of Illinois in the following page:

However, it is always advised that you consult a lawyer to avoid any dispute that may arise in future due to wrong filing of the deed.
