doesn't your employer have guidelines?
maybe you are a freelance processor - is that the case?
i truly don't quite understand what would lead you to this site to ask for this information, unless you are an unemployed processor.

Hi,
Beside the good faith estimate the lender must provide disclosures within three business days like Truth in lending Act statement, Servicing disclosure statement and in addition to these if someone applies from a mortgage company which is operated by a real estate agent he should get a Affiliated business arrangement disclosure.
