Why is my bank contacting me 3 yrs. later to sign a PUD agreement they said was not signed initially?

Well, this is something that your bank will be able to answer in a better manner. You should contact them and ask them to clarify the matter.

They have obviously belatedly discovered that a document didn't get signed. You would have signed a specific disclosure at your closing that allows for this sort of situation to be rectified if and when discovered. The timing might seem unusual, but perhaps there's been a recent audit and the lack of such a disclosure was discovered. Inasmuch as you've already consented at the closing to fix paperwork problems, you would need to comply with their request. It's more of a technicality, no doubt, than anything else; why not do as they request and be cooperative?
