I am in need of a waiver form for my husband to sign as I am looking to buy proerty. Does anyone have such a form? Thanks Theresa

Well... I couldn't find such a waiver form. Nevertheless, I will suggest you to contact an attorney and get the waiver form drafted from him. The attorney will be the best to help you in this regard.

You need a legal assistance since we are dealing with important documents fro purchasing a property like what you said. I don't have such form but try asking your local government if they can help you with your problem.
