Are you a loan officer?
If so you need to have origination software. I use Calyx Point software and I get all the disclosures I need from there. Any special disclosures can be loaded in by an administrator at your company.

Eric is right that if you are a loan officer that your company LOS software should provide this to you. If you are a borrower, then I would suggest going to the state of Maryland website and they should have the information you need.
