Death of joint tenant

Hello, my name is Dave and have a question regarding removing a name from a deed. My parents own their house together as joint tenant. My mother just passed away last month and my father wants to make a living trust. In order to do that I believe he has to remove my mother from the deed, is that correct? I also was reading in california you have to file an affidavit of Death of joint tenant. I printed out the form and not exactly sure how to fill it out. I've been searching around the web for help and thats how I found this forum. Can the form be filled out by myself or do I need a lawyer?? Last question, toward the bottom there is an area to fill in what the property is worth, do I put the purchase price back when they purchased it, or the current market value, will this have any affect on his future property taxes or do they allow x amount of $$ they are allowed. I would really appreciate any advise I can get.
yes dave. as a first step your father need to remove name of your mother, he can do it by filing an affidavit of Death of joint tenant.

then he can form a living trust.
