How do I get "The estate of my husband" off the mortgage note?

Property jointly owned. No estate opened. Mortgage company in process of lowering house payment. Wants property in my name only. County clerk said get new deed form from office supply, have notarized, and submit with $10.00. Already sent death certificate at cost of $10 to register with clerk of court. No such form found at office supply store. Pleasse advise proper form to get. Thanks
