i just found out that it means Center for Primary Care and Outcomes Research.

PCOR stands for Preliminary Change of Ownership Report. As per the laws in certain states, new homeowners and anyone holding an interest in property are required to file a PCOR with the County Recorder's office. This should be done at the time of recording a deed.

The Report is usually filled out with the help of escrow companies at the time of home buying. When a PCOR accompanies the deed, the County Recorder does not charge a fee along with the recording of the deed.

The Preliminary Change of Ownership Report helps a property assessor determine whether or not there has been any change in the fair market value at the time of change of ownership.
