Hi,
The Notarization section of the Quitclaim Deed contains the following:
"On---------------, before me,----------------------------------------------------
appeared-------------------------------------------------------,person ally known to me (or proved to me on the basis of satisfactory evidence) to be the person(s) whose names(s) is/are subscribed to the within instrument and acknowledged to me that he/she/they executed the same in his/her/their authorized capacity(ies), and that by his/her/their signature(s) on the instrument the person(s), or the entity upon behalf of which the person(s) acted, executed the instrument."
Kindly advise what to put in the blank spaces.
Thanks/Regards

As much as i understand, i think the first blank should have the date when you are executing the deed. The second blank space should have name of the person who is conveying the property to you.
I'm not a legal professional, so I'd say that you consult an attorney. |

| The notary needs to fill this section out. It should be "On (date that document was signed and notarized), before me, (notary's name) appeared (names of all persons that signed the deed)" |
