What wording is used on the quit claim deed to transfer the property out of his name as the trustee of the trust and into both our names as husband and wife joint tenants? and do we need to record a copy of the trust and death certificate as well as the deed?

The deed, death certificate and other documents have to be recorded at the county recorder's office.
