Which is proper Texas quit claim to use when a couple divorces and there is a mortgage?

What is the exact name of the form to be filed in Texas to remove the former spouse from a deed when there is almost no equity and the other spouse will assume the mortgage?

The former spouse will have to sign a quitclaim deed in the name of the other spouse and transfer the property to him or her. Then the other spouse can refinance/assume the mortgage in his or her name.
