Smith and Suzy M. Smith as joint tenants (for example) then under "hereby remise, quitclaim to John M. Smith and Suzy M. Smith, trustees of the Smith Family Trust dated September 15, 2010? What do I have to fill out on the "preliminary Change of Ownership report" ? Tx

In my opinion, you should contact your real estate attorney in order to fill out the "preliminary Change of Ownership report". He will guide you in filling out the form in a correct way.
