If the trust is an individual trust then you can transfer property as being its trustee on your own. But if it is a shared trust, you will have to get consent of any co-trustee before transferring the property. Trustee will use a quit claim deed to transfer property held in the trust to an individual.

Dourdir, trustee will be the grantor and the individual getting property will be named as grantee on the deed.
