cmorgan, the only way in which you can be eliminated from the mortgage note is if your partner refinances the loan and obtains a new mortgage. you are obligated currently until the loan is paid in full, and that would remain the case unless and until he refinances.

there is such a thing as "novation" that a lender may approve, but in that case, your partner would need to be qualified to carry the loan by himself.

Yes, I agree that refinancing is the most reasonable way for you to do this. If your partner wants to keep the home, then he or she will refinance the loan. If neither one of you want to keep the home, then of course there other options like selling the property.
