Who is the real owner?

In a situation where a house owner, who is still on the loan, has been misled and has unintentionally signed the title over to someone else, who is the real owner? Does the loan-holder have any rights?

The person to whom the title has been signed over will be considered as the real owner of the property. The loan holder does not have any rights to the property.
