My mother is 89 years old. She owns a home that is all paid off and wants to put my name on the deed so it is mine when she dies. She is in the Star program and I was told by them that my name on the deed will not affect the program because she lives there and I don't.

As the property is free and clear, your mother can use a warranty deed in order to transfer the property to you. You can contact an attorney and get the property deed drafted from him. Once your mother fills out the deed, you'll have to record it at the county recorder's office.
