If relative wants to add us to his mortgage and deed, (we would make the payments and live in the house, he would move) what steps would we need to take?

You will have to ask the relative to sign a quit claim deed and add your names to the property deed. Then, you can refinance the mortgage and add your names to the loan docs. This will make you responsible for the mortgage payments as well.
