I also changed my last name and want that reflected on the deed.
You will have to have a grant deed written up, and both you and your ex husband will have to sign.

ashleyblanton is right. You can draw up a grant deed. But you can also do it using a quitclaim deed. In that case, your ex-husband needs to sign the deed as the grantor giving up his interest in the property. In order to have your new last name on the deed, you can use quit claim deed to transfer your share as a grantor (your old last name) to you as grantee (your new last name).
