my mom filed a transfer on death deed leaving her home to my brother and myself upon her death.
when she died, we filed an affidavit of survivorship and the deed was changed to my brother and my names.
my question is: how do we now go about "adding" my other brother's name onto the deed?
the house is free and clear so i am thinking a quit claim deed would suffice.

You and your brother can use a quit claim deed in order to add your second brother to the property deed. However, as the house is free and clear, you will be able to use the warranty deed as well.

What are my tax obligations when I file a quit claim deed in Ohio?
I know I have to pay the recording fee but when I spoke to the auditors office, they said that "add ons were exempt" and that I would only have to pay a 50 cent transfer fee. Would that be 50 cents per thousand or just a one time total of 50 cents ???

As far as I can understand, you will have to pay 50 cent per thousand. You should contact the county recorder's office and ask them to clarify the details.
