How does one go about adding girlfriends name to deed of recently purchased home? And will it change the rate if her credit is not so great?

You can add your girlfriend's name on the property deed through a quit claim deed. You need to sign the deed as grantor and both you and your girlfriend would be grantees. You need to notarize the deed and record it with the county recorders office to validate it. Adding her to the title will not change the interest rate on the mortgage.
