Bank did a modification they told me not to worry about the 2nd since chase carried both i checked my credit and the 2nd says paid/closed but it is still on title how do i remove it off title and did they forget to record it

you should first check out with your lender whether or not they have forgiven the loan. your lender will be able to let you know what they have done with your second mortgage. if the lender has closed the account, then you may ask him to give you a lien release certificate for it.
